Appointment Request

At Focus Eye Care we are pleased to offer online appointment scheduling for your annual eye exam! If you are a new patient to our clinic you will need to text our office your name and email address first in order to access the online portal. If you are existing simply enter your email address we have on file. 

Click the link below to schedule an appointment online with either of our eye doctors. You will be directed to our patient portal where you will use your email to login and then select your date/time.  You may also call our office at (515) 264-2445 to schedule same day appointments. At this time our online portal does not allow same day scheduling but we frequently are able to see you that day, so call us to inquire about an appointment!
